Chapter 6: Taking Action

This chapter covers program planning—specifically prioritizing and developing an action plan and program model. Recognizing that coalitions must decide how to use information from the needs-and-assets assessment to address the selected Objectives, this chapter provides guidance for designing an intervention—from identifying broad goals and objectives to developing specific timetables and detailing resources needed to implement the intervention. It also encourages communities to develop a logic model (or “program model”), a tool that shows the links between the needs and the target populations, the goals and objectives, and the intervention strategies. The logic model can also be used as a foundation for the evaluation and to facilitate program planning and implementation. Worksheets are provided to help guide each phase of the planning.
