Core Partners - Registration & Evaluation Forms
- Core Partner Registration Form
- Form 1: Materials Dissemination Log for Core Partners: This form allows Core Partners to keep a running log of organizations and individuals to whom they pass along National Initiative Materials.
- Form 2: Activity & Feedback Form for Core Partners: This form will help us learn from Core Partners' organizations' utilization of National Initiative materials. We encourage Core Partners to add any details or information of creative presentations, activities, etc. that they wish.
Other Professionals Engaged in the National Initiative - Evaluation Form
- Form 3: National Initiative Adolescent Health Materials Response Form: This form will be used to gain feedback directly from Other Professionals engaged in the National Initiative whom receive National Initiative materials.
