The Adolescent and Young Adult Health National Resource Center and core partner, the National Improvement Partnership Network (NIPN) at the University of Vermont, are delighted to announce that the Adolescent and Young Adult Health Measure Set is complete and available for use. These are a set of standard process measures that can be used to assess and track improvement within and among pediatric and family practices. This Adolescent and Young Adult Measure Set is intended to be used as a reference tool to provide guidance to practices measuring improvement in health care quality specific to adolescents and young adults.
