The National Improvement Partnership Network (NIPN) at the University of Vermont, a core partner of the Adolescent and Young Adult Health National Resource Center, developed a set of Adolescent and Young Adult Health Measure Set as part of an ongoing series of measures for quality improvement initiatives. These are a set of standard process measures that can be used to assess and track improvement within and among pediatric and family practices. This Adolescent and Young Adult Measure Set is intended to be used as a reference tool to provide guidance to practices measuring improvement in health care quality specific to adolescents and young adults.
